Background:

  • Cutaneous squamous cell carcinoma (cSCC) is a common skin malignancy.
  • Rare variants of cSCC present diagnostic challenges.

Observation:

  • A 50-year-old man presented with a neck lesion exhibiting intra-epidermal and invasive dermal cells with a signet-ring appearance.
  • Histopathology revealed features suggestive of cSCC, with signet-ring morphology.
  • Immunohistochemical staining demonstrated keratin expression and focal glycogen positivity, with negative mucicarmine stains.

Findings:

  • The lesion was diagnosed as signet-ring squamous cell carcinoma based on unique cellular morphology and immunohistochemical profile.
  • Ultrastructural examination identified dilated endoplasmic reticulum as the source of the signet-ring vacuoles.
