A rare vascular tumor presented as a bleeding mass in a 60-year-old man. Surgical excision confirmed a benign vascular lesion, not a sarcoma.

Area of Science:

  • Dermatology
  • Surgical Pathology
  • Vascular Tumors

Background:

  • A 60-year-old male with cardiac insufficiency presented with a 5-year history of a slowly growing, bleeding tumor on his right hypochondrium.
  • The lesion developed a superficial ulcer and intermittent bleeding 6 months prior to consultation, with no associated pain.

Observation:

  • Dermatologic examination revealed a violaceous, ulcerated, and bleeding tumoral lesion, approximately 3-4 cm in size.
  • The mass was hemispheric, non-tender, adherent to the epidermis, and movable on underlying tissue.

Findings:

  • Incisional biopsy initially suggested dermatofibrosarcoma protuberans or desmoid tumor.
  • Histopathology revealed prominent blood-filled vascular spaces with PAS-positive material and two cell types.

Related Experiment Videos

  • Crucially, no malignant transformation was identified, indicating a benign vascular neoplasm.
  • Implications:

    • This case highlights the importance of thorough histopathological examination to differentiate benign vascular tumors from potentially malignant lesions.
    • Accurate diagnosis is critical for appropriate management and patient prognosis.
    • The successful total excision and primary suturing demonstrate effective treatment for this type of lesion.
