Summary

Emergency peri-partum hysterectomy is a rare but life-saving procedure for obstetric hemorrhage. While associated with significant morbidity, this study found no maternal deaths in 14 cases over 10 years.

Background:

  • Hysterectomy during cesarean or postpartum is a critical intervention for severe obstetric hemorrhage.

Purpose of the Study:

  • To review cases of cesarean and postpartum hysterectomy over a 10-year period.
  • To specifically analyze the associated morbidity in these procedures.

Main Methods:

  • Retrospective review of case files from a teaching hospital.
  • Analysis of 14 cases of cesarean and postpartum hysterectomy performed between 1986 and 1996.

Main Results:

  • Cesarean hysterectomy occurred in 0.17% of cesarean sections; postpartum hysterectomy after vaginal delivery in 0.02%.
  • Uterine atony (43%), placenta accreta/increta (50%), and placenta previa (57%) were common indications.
  • Postoperative morbidity included fever (28%), urinary tract infection (21%), and chest infection/atelectasis (21%). No maternal deaths were reported.

Conclusions:

  • Emergency peri-partum hysterectomy for obstetric hemorrhage is rare (1 in 2,550 deliveries).
  • Despite intraoperative risks and postoperative morbidity, it is a potentially life-saving procedure.
