Summary

Electroconvulsive therapy (ECT) with suxamethonium does not increase intraocular pressure (IOP) beyond expected levels in psychiatric patients. This suggests ECT is safe for patients on medications that can cause glaucoma.

Background:

  • Prolonged use of phenothiazine, tricyclic antidepressants, and antiparkinsonian drugs can lead to mydriasis and angle closure glaucoma in psychiatric patients.
  • Suxamethonium administration during electroconvulsive therapy (ECT) transiently increases intraocular pressure (IOP).

Purpose of the Study:

  • To investigate the effects of electrically induced convulsions, following anesthesia with methohexitone and suxamethonium, on intraocular pressure (IOP) in psychiatric patients.
  • To assess the ocular safety of ECT in psychiatric patients undergoing treatment with antipsychotropic medications.

Main Methods:

  • 21 cooperative psychiatric patients receiving antipsychotropic drugs were studied.
  • Anesthesia was induced with methohexitone (1 mg.kg-1) followed by suxamethonium (0.5 mg.kg-1).
  • Intraocular pressure (IOP) was measured sequentially before anesthesia, after loss of eyelash reflex, after suxamethonium fasciculations, after convulsion, and after spontaneous respiration.

Main Results:

  • Methohexitone significantly reduced IOP from a baseline of 15.3 mmHg to 13.5 mmHg.
  • Suxamethonium increased IOP to 16.1 mmHg, and the subsequent electrically induced convulsion further increased IOP to a peak of 19.2 mmHg.
  • IOP returned to near baseline levels (15.5 mmHg) after spontaneous respiration resumed; all changes were statistically significant (p < 0.001).

Conclusions:

  • The IOP reduction by methohexitone was reversed by the IOP increase from suxamethonium.
  • The IOP increase following the induced convulsion was comparable to that expected from suxamethonium alone.
  • Electrically induced convulsions during ECT do not appear to pose an additional ocular hazard to psychiatric patients on medications with potential glaucomatous side effects.
