Mannan-binding lectin (MBL) insufficiency is linked to rheumatoid arthritis (RA) development and progression. Lower MBL levels in RA patients correlate with earlier onset and more severe disease indicators.

Background:

  • Low serum mannan-binding lectin (MBL) levels are associated with increased susceptibility to infections.
  • Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by joint inflammation and destruction.

Purpose of the Study:

  • To investigate the association between serum MBL levels and the clinical course and prognosis of rheumatoid arthritis (RA).

Main Methods:

  • Serum MBL levels were quantified in 99 RA patients enrolled in a long-term prospective study.
  • Patient data including age at onset, erythrocyte sedimentation rate, joint swelling, joint motion limitation, and radiographic destruction were analyzed.

Main Results:

  • A significantly higher proportion of RA patients had undetectable MBL compared to controls (11% vs 3%, p=0.025).

  • Patients with MBL levels below the median exhibited younger age at RA onset (p=0.043).
  • Lower MBL levels were associated with elevated erythrocyte sedimentation rate (p=0.006), increased joint swelling (p=0.019), greater limitation of joint motion (p=0.027), and a higher annual increase in radiographic destruction (p=0.053).
  • Conclusions:

    • MBL insufficiency may play a role in the pathogenesis of rheumatoid arthritis.
    • Serum MBL levels could serve as a potential biomarker for RA disease activity and progression.
