Summary

Guided tissue regeneration (GTR) for severe periodontitis can lead to progressive cervical root resorption. Histological analysis revealed extensive resorption cavities on the root surface, indicating a potential complication of regenerative periodontal therapy.

Background:

  • Root resorption, ankylosis, and alveolar bone loss are known complications following periodontal treatments.
  • Guided tissue regeneration (GTR) is a common periodontal regenerative therapy.

Observation:

  • A 46-year-old woman with severe periodontitis underwent periodontal regenerative treatment with GTR for angular defects.
  • Two years post-treatment, severe proximal cervical root resorptions were observed on tooth 25, with no signs of root caries.

Findings:

  • Histological analysis of the extracted root of tooth 25 revealed extensive resorption cavities on the cervical and proximal root surfaces.
  • These cavities were lined with CD68-positive mononuclear and multinucleated cells and penetrated into the root canal.

  • This case highlights progressive cervical root resorption as a potential sequela of GTR in periodontal treatment.
  • Further research is needed to understand the mechanisms and prevention strategies for GTR-associated root resorption.
