Summary

Elevated non-esterified fatty acids (NEFA) enhance insulin secretion during hyperglycemia in both healthy individuals and those at risk for Type 2 diabetes. This suggests NEFA plays a role in modulating insulin response.

Background:

  • Elevated non-esterified fatty acid (NEFA) levels are implicated in insulin resistance and Type 2 diabetes mellitus (T2DM) development.
  • Understanding the acute effects of NEFA on insulin secretion is crucial for elucidating T2DM pathogenesis.

Purpose of the Study:

  • To investigate the impact of acute NEFA elevation on insulin secretion in response to glucose and arginine challenges.
  • To compare these effects in healthy controls and individuals with a predisposition to T2DM.

Main Methods:

  • A crossover study design was employed with triglyceride (TG) plus heparin infusion to elevate NEFA levels, contrasted with a saline control infusion.
  • Subjects underwent glucose bolus, a 90-minute hyperglycemic clamp, and arginine stimulation during both conditions.

  • Insulin and C-peptide levels were measured throughout the study in 6 controls and 6 T2DM-risk subjects.

    • Acute NEFA elevation did not alter basal or first-phase glucose-stimulated insulin levels.

    • Acute elevation of NEFA enhances both glucose- and non-glucose-stimulated insulin secretion.
    • This suggests a mechanism by which NEFA may modulate pancreatic beta-cell function in the context of metabolic challenges.
