Growth hormone (GH) treatment prevents reduced insulin-like growth factor I (IGF-I) gene expression in skeletal muscle following major surgery. This finding is crucial for understanding and managing protein catabolism in surgical patients.

Background:

  • Major surgery, sepsis, and trauma increase protein catabolism, leading to acquired growth hormone (GH) resistance and reduced skeletal muscle mass.
  • GH treatment and glutamine-containing parenteral nutrition can mitigate this catabolism.
  • GH effects are partly mediated by insulin-like growth factor I (IGF-I), produced in the liver and target tissues.

Purpose of the Study:

  • To investigate the impact of GH administration on IGF-I and GH receptor (GHR) gene expression in skeletal muscle after major abdominal surgery.
  • To establish a quantitative RT-PCR assay for measuring IGF-I gene expression in muscle tissue.

    • In the control group, IGF-I mRNA levels significantly decreased from preoperative levels to postoperative day 3 (P < 0.05).
    • GH-treated patients showed no significant change in IGF-I mRNA levels between preoperative and postoperative samples.

    Conclusions:

    • GH administration effectively prevents the decline in IGF-I gene expression observed in skeletal muscle after major abdominal surgery.
    • This suggests a mechanism by which GH may counteract surgical-induced muscle catabolism.
