Background:

Purpose of the Study:

  • To determine the incidence of eyelid malpositions after cataract extraction.
  • To identify etiological factors contributing to post-cataract ptosis.

Main Methods:

  • A prospective study followed 124 patients for six months post-cataract extraction.
  • Preoperative and postoperative measurements included palpebral aperture, levator function, lid crease height, and lower eyelid laxity.
  • Post-cataract ptosis was defined as a ≥2 mm drop in the lid margin.

Main Results:

  • No ectropion or entropion occurred; 4% developed punctal ectropion.
  • The incidence of post-cataract ptosis was 7.3% at six months.
  • Preoperative factors were not predictive, but preoperative palpebral fissure width was significant (p<0.05).
  • Local anesthetic volume correlated with ptosis on postoperative day one (p<0.05).
  • Ptosis on postoperative day one strongly predicted six-month ptosis (p<0.001).

Conclusions:

  • Multiple factors contribute to post-cataract ptosis.
  • Temporary ptosis may result from local anesthetic myotoxicity.
  • Early ptosis detection is the most reliable predictor of long-term outcomes.
