Evidence is presented for the efficacy and effectiveness of cardiac rehabilitation programmes. A three elemental model for cardiac rehabilitation is introduced. Key recommendations are made for hospitals running cardiac rehabilitation programmes. Directions for the future development of cardiac rehabilitation are suggested
