Summary

Transient global amnesia (TGA) occurred in a patient after mandibular cyst surgery. While long-term memory returned, recent memory remained lost, suggesting TGA as a potential complication.

Area of Science:

  • Neurology
  • Anesthesiology
  • Oral and Maxillofacial Surgery

Background:

  • A 61-year-old male underwent marsupialization for a left mandibular cyst under general anesthesia.
  • The procedure involved surgical intervention within the mandibular region.

Observation:

  • Post-anesthesia, the patient exhibited impaired memory for the preceding four months and short-term memory plasticity.
  • No immediate neurological abnormalities were detected.
  • On postoperative day two, retrograde memory for the past four months was restored.

Findings:

  • A distinct period of amnesia persisted, encompassing events from six hours pre-operation to the morning after.
  • This memory deficit is hypothesized to be transient global amnesia (TGA).

Implications:

  • Transient global amnesia may be a potential complication following mandibular procedures under general anesthesia.
  • Possible contributing factors include transient hypertension, operative stress, postoperative pain, and diazepam administration.
  • Further research is warranted to elucidate the specific mechanisms and risk factors for TGA in this context.
