Area of Science:

  • Obstetrics and Gynecology
  • Perinatal Medicine
  • Reproductive Health

Background:

  • Breech presentation complicates delivery, influencing obstetric decisions.
  • Understanding factors affecting delivery method is crucial for maternal and infant outcomes.

Purpose of the Study:

  • To investigate factors influencing the choice of delivery method for breech presentation.
  • To analyze trends in cesarean section rates for breech births over time.

Main Methods:

  • Population-based cohort study utilizing The Medical Birth Registry of Norway.
  • Inclusion of all singleton deliveries in Norway from 1967 to 1994.
  • Analysis of 1,592,064 deliveries, with 45,921 (2.9%) in breech presentation.

Main Results:

Related Experiment Videos

  • Cesarean section proportion for breech presentation rose from 3.6% (1969) to 58.8% (1994).
  • Relative risk of cesarean section for breech births increased from 2.0 to 5.9 compared to non-breech.
  • Cesarean rates declined with maternal age, increased with gestational age, and were lower in urban residences.

Conclusions:

  • Vaginal breech delivery was associated with large delivery departments and urban maternal residence.
  • Centralization of breech deliveries to experienced centers appears justified for successful vaginal birth.
  • Recommendations include pursuing further centralization and establishing preparedness for vaginal breech delivery in smaller units through training.
