    Background:

    • Occluded or inadequate pupillary space presents surgical challenges.
    • Formed vitreous in the anterior chamber can lead to complications in aphakic eyes.

    Purpose of the Study:

    • To evaluate the efficacy of vitreoretinal surgical instrumentation and techniques for anterior segment issues.
    • To treat eyes with pupillary space obstruction and vitreous complications in the anterior chamber.

    Main Methods:

    • Utilized instrumentation and techniques developed for vitreoretinal surgery.
    • Applied a pars plana approach in 19 eyes and a limbus approach in 6 eyes.
    • Successfully removed formed vitreous from the anterior segment in all treated eyes.

    Main Results:

    • Achieved adequate pupillary space in all treated eyes.
    • Complete removal of formed vitreous from the anterior segment was accomplished.
    • No intraoperative complications were reported during the procedures.

    Conclusions:

    • This adapted approach offers advantages for selected anterior segment surgeries.
    • Further studies with more cases and longer follow-up are needed to define indications and limitations.
