Mitochondrial respiratory chain defects can cause neurological disorders mimicking multiple sclerosis. Investigating mitochondrial encephalomyopathy is crucial for patients with atypical neurological presentations.

Background:

  • Mitochondrial respiratory chain defects lead to diverse clinical disorders, often presenting as non-specific encephalopathies.

  • Two patients presented with neurological syndromes clinically indistinguishable from multiple sclerosis.
  • Magnetic resonance imaging findings were consistent with multiple sclerosis in both cases.

Findings:

  • Muscle biochemistry revealed defects in complex IV of the mitochondrial respiratory chain.
  • The patients were diagnosed with mitochondrial encephalomyopathy.
