Area of Science:

  • Gastroenterology
  • Digestive Endoscopy
  • Physiology

Context:

  • Sphincter of Oddi dysfunction (SOD) poses diagnostic challenges.
  • Endoscopic manometry is the established gold standard for SOD diagnosis.
  • Limited data exists on the initial implementation of this technique in clinical practice.

Purpose:

  • To present the outcomes of the first 30 endoscopic manometries for sphincter of Oddi performed in a specialized Gastroenterology Service.
  • To evaluate the safety and diagnostic yield of endoscopic manometry in a real-world clinical setting.

Summary:

  • Thirty endoscopic manometries were performed in 28 patients (30-70 years old).
  • Successful deep papilla cannulation was achieved in 88% with no observed complications.
  • Manometry identified normal function in 11 patients, hypertonic dyskinesis in 5, and variable results post-papillotomy, highlighting its diagnostic capability for SOD subtypes.

Impact:

  • Establishes endoscopic manometry as a safe and effective diagnostic method for sphincter of Oddi functional disorders.
  • Provides baseline data for future studies on SOD management.
  • Enhances clinical decision-making in patients with suspected biliary and pancreatic pain.
