Background:

  • T-cell lymphoma is a rare malignancy.

Observation:

  • A 63-year-old woman presented with acute bilateral perilimbal congestion and chemosis.
  • Corneal infiltration and thickening developed, alongside an incidentally discovered maxillary sinus mass.

Findings:

  • Histopathologic and immunohistochemical analysis confirmed diffuse large B-cell lymphoma, stage IV.
  • The patient achieved remission with chemotherapy and 13-cis-retinoic acid.

Implications:

  • Conjunctival T-cell lymphoma can present insidiously, mimicking inflammatory conditions like episcleritis.
