A rare case of extramedullary plasmacytoma in the maxillary sinus was identified. This locally aggressive tumor mimicked adenocarcinoma but was confirmed via immunohistochemistry and light chain restriction, ruling out multiple myeloma.

Area of Science:

  • Oncology
  • Pathology
  • Head and Neck Cancer

Background:

  • Extramedullary plasmacytomas are rare plasma cell neoplasms occurring outside the bone marrow.
  • Maxillary sinus involvement is uncommon, presenting diagnostic challenges.
  • Distinguishing these from carcinomas is crucial for appropriate treatment.

Observation:

  • A 65-year-old male presented with a locally aggressive maxillary sinus mass.
  • Initial clinical suspicion was adenocarcinoma.
  • Pathological examination revealed anaplastic morphology with limited plasmacytic differentiation.

Findings:

  • Immunohistochemistry showed a lack of cytokeratins and epithelial membrane antigen (EMA).
  • Light chain restriction confirmed plasmacytic origin.

Related Experiment Videos

  • Absence of multiple myeloma criteria supported the diagnosis of extramedullary plasmacytoma.
  • Implications:

    • Accurate diagnosis of extramedullary plasmacytoma is vital for treatment planning.
    • Immunohistochemistry plays a key role in differentiating from epithelial malignancies.
    • This case highlights the importance of comprehensive pathological evaluation in challenging head and neck lesions.
