Summary

Mitomycin trabeculectomy induced more corneal astigmatism than conventional surgery in glaucoma patients. This effect may stem from impaired wound healing and lower intraocular pressure with mitomycin use.

Purpose of the Study:

  • To compare the impact of conventional trabeculectomy versus mitomycin-assisted trabeculectomy on surgically-induced corneal astigmatism.
  • To evaluate the changes in corneal astigmatism over time following these procedures.

Main Methods:

  • Computer-assisted videokeratography (Eye-Sys) was used to measure corneal astigmatism.
  • 34 glaucoma patients undergoing trabeculectomy were analyzed.
  • Color-coded topography maps provided astigmatic data.

Main Results:

  • Mitomycin trabeculectomy resulted in higher surgically-induced astigmatism (4.0 D) compared to conventional trabeculectomy (2.2 D) at 1 day post-operation.
  • Astigmatism decreased over time but remained higher in the mitomycin group (1.1 D vs. 0.4 D at 3 months).

Conclusions:

  • A correlation exists between mitomycin use and increased induced astigmatism.
  • Impaired wound healing and lower intraocular pressure in the mitomycin group may contribute to this finding.
