Background:

  • Acquired palmar spiny keratoderma (SK) is a rare skin condition.
  • Type IV hyperlipoproteinemia is a metabolic disorder characterized by elevated triglycerides and VLDL.

Observation:

  • A 35-year-old man presented with acquired palmar spiny keratoderma (SK) developing over 11 years.
  • No history of predisposing factors like topical treatments, Vitamin A derivatives, radiation, arsenic, or sun exposure.
  • Blood tests revealed hypertriglyceridemia and elevated VLDL, consistent with type IV hyperlipoproteinemia.

Findings:

  • Histological examination of SK lesions showed columnar hyperparakeratosis with a hypogranular layer.
