Summary

Surgical outcomes have improved for elective procedures, but emergency cases still have high mortality. Better understanding of intensive care unit (ICU) physiology is key to improving survival in critical surgical patients.

Area of Science:

  • Cardiovascular Surgery
  • Critical Care Medicine

Context:

  • Elective surgery mortality rates have significantly decreased to below 5% in many centers.
  • Emergency surgical cases continue to exhibit high mortality rates, often exceeding 50%.

Purpose:

  • To analyze the evolving clinical presentation of critically ill surgical patients.
  • To discuss organizational and technical advancements improving surgical outcomes.

Summary:

  • While elective surgery results have dramatically improved, emergency surgery outcomes remain poor.
  • The clinical picture has shifted, with most deaths occurring post-operatively in the Intensive Care Unit (ICU) rather than the Operating Room.
  • Enhanced survival in emergency surgical cases depends on a deeper understanding of ICU-related physiological derangements.

Impact:

  • Improved understanding of ICU physiology can enhance survival rates for high-risk surgical patients.
  • Organizational and technical changes in surgical practice contribute to better patient outcomes.
