Purpose of the Study:

  • To investigate the direction of juxtapyloric duodenal pressure waves post-meal.
  • To compare postprandial duodenal contractions with interdigestive phase II contractions.

Main Methods:

  • Antroduodenal manometry in 11 healthy subjects.
  • Analysis of antegrade and retrograde pressure wave propagation in four duodenal subsegments (15 mm each) over a 6-cm segment.
  • Motility recording for 60 minutes after a test meal.

Main Results:

  • Postprandially, 40-50% of duodenal pressure waves were retrograde in the D1-D2 subsegment (juxtapyloric), significantly higher than in the D4-D5 subsegment (29-10%).
  • In contrast, interdigestive phase II showed predominantly antegrade pressure waves across all duodenal subsegments.
  • Retrograde waves were significantly more prevalent in the juxtapyloric region post-meal (P < 0.05 and 0.01).

Conclusions:

  • A significant proportion (40-50%) of duodenal pressure waves exhibit retrograde propagation in the juxtapyloric area after meals.
  • Antegrade contractions dominate further down the duodenum (5-6 cm distal to pylorus) postprandially.
  • Retrograde duodenogastric propulsion is a key factor influencing gastric emptying rate.
