Background:

  • Parkinson's disease (PD) is a progressive neurodegenerative disorder.
  • Intractable motor symptoms in PD pose significant treatment challenges.
  • Electroconvulsive therapy (ECT) is primarily used for severe depression.

Observation:

  • Four non-depressed, non-demented patients with intractable PD received ECT.
  • An acute ECT phase was followed by maintenance bitemporal ECT every 3-4 weeks for up to 12 months.
  • Mood, cognition, and motor function were serially assessed.

Findings:

  • Two patients completed 12 months of maintenance ECT.
  • These patients reported significant reductions in "off" time (motor fluctuations).
  • Cognitive function remained unimpaired in those completing the study.
