Ectopia lentis et pupillae (ELeP) is a rare disorder of the eye. High-resolution ultrasound imaging revealed structural abnormalities, suggesting mechanical tethering causes ELeP.

Background:

  • Ectopia lentis et pupillae (ELeP) is a rare congenital inherited disorder.
  • Pathogenesis theories include neuroectodermal defects or persistent fetal vasculature.
  • Previous studies lacked histopathology or advanced imaging.

Observation:

  • A 55-year-old woman with ELeP underwent 10-MHz and 50-MHz ultrasonography.
  • Pupillary displacement was observed: inferiorly on the right, temporally on the left.
  • Ultrasound revealed lens displacement and structural abnormalities in the anterior segment.

Findings:

  • 50-MHz ultrasound provided high-resolution in vivo imaging of anterior structures.
  • Key findings included undefined ciliary processes and a membrane tethering the pupil.
  • These structural abnormalities suggest mechanical factors in ELeP pathogenesis.

Implications:

  • Findings differ from non-ELeP-related aphakia, indicating a specific etiology for ELeP.
  • The results support a hypothesis of mechanical tethering and zonular disruption.
  • This supports a novel understanding of ELeP pathogenesis, distinct from recent theories.
