This study describes a rare case of trilateral retinoblastoma. Elevated lactic dehydrogenase (LDH) in cerebrospinal fluid (CSF) may indicate intracranial tumors in retinoblastoma patients.

Background:

  • Trilateral retinoblastoma, a rare condition involving bilateral retinoblastoma and an intracranial primitive neuroectodermal tumor, requires early detection.
  • Standard tumor markers (hCG, AFP, CEA) are often unhelpful in diagnosing intracranial extension.
  • This case highlights the diagnostic challenges in sporadic retinoblastoma with potential central nervous system involvement.

Observation:

  • A 20-month-old male presented with a sporadic retinoblastoma.
  • Clinical and imaging evaluations were performed.
  • Blood and cerebrospinal fluid (CSF) were analyzed for tumor markers: human chorionic gonadotrophin (hCG), alpha-feto protein (AFP), carcinoembryonic antigen (CEA), and lactic dehydrogenase (LDH).

Findings:

  • Only lactic dehydrogenase (LDH) levels were elevated in the cerebrospinal fluid (CSF).
  • Other tested tumor markers (hCG, AFP, CEA) in blood and CSF were within normal limits.
  • The retinoblastoma gene could not be isolated in this patient.

Implications:

  • Elevated CSF LDH may serve as a potential biomarker for intracranial involvement in retinoblastoma.
  • Routine CSF LDH analysis is proposed for bilateral retinoblastoma patients with initially normal brain scans.
  • Periodic non-invasive brain imaging is recommended for patients with elevated CSF LDH to enable early detection of intracranial tumors.
