Summary

Adrenal function in chronic renal failure (CRF) patients is generally unaffected by hemodialysis or peritoneal dialysis. Low-dose adrenocorticotropin (ACTH) testing confirmed normal adrenal response in CRF patients, regardless of renal replacement therapy type.

Background:

  • Conflicting findings exist regarding hypothalamic-pituitary-adrenal axis function in chronic renal failure (CRF).
  • Low-dose adrenocorticotropin (ACTH) testing may reveal subtle adrenal dysfunction missed by standard doses.
  • Renal replacement therapy modality (hemodialysis vs. CAPD) could influence adrenal function.

Purpose of the Study:

  • To evaluate adrenal function in CRF patients using varying doses of ACTH.
  • To determine if hemodialysis or continuous ambulatory peritoneal dialysis (CAPD) impacts adrenal response.
  • To investigate the utility of low-dose ACTH testing in CRF.

Main Methods:

  • ACTH stimulation tests were conducted using 1, 5, and 250 microg doses.
  • Participants included 14 CRF patients (7 on hemodialysis, 7 on CAPD) and 7 healthy controls.
