Background:

  • Peripheral vascular disease (PVD) involves abnormalities in peripheral blood vessels, causing irregular blood flow.
  • Symptoms often manifest as ischemia or inflammation of affected vessels.

Observation:

  • A previously healthy 4.5-year-old girl presented with digital bruises, abdominal pain, fever, and facial swelling.
  • Physical examination revealed severe hypertension, absent peripheral pulses, and extensive finger necrosis leading to dry gangrene.
  • Digital subtraction angiography (DSA) demonstrated segmental arterial obstruction.

Findings:

  • Despite high-dose immunosuppressants (methylprednisolone, cyclophosphamide) and antiplatelet agents (salicylates, dipyridamole), symptoms persisted.
  • A thoracic sympathectomy was performed, leading to improved peripheral circulation.
  • However, a demarcation zone on the fingertips necessitated amputation.

Implications:

  • This case highlights the critical need for early diagnosis and aggressive management of pediatric PVD.
  • It underscores the potential for severe vascular complications even in previously healthy children.
  • Treatment strategies may require a multidisciplinary approach, including surgical intervention.
