Background:

Sphincter-saving deep anterior rectal resection enhances quality of life for rectal cancer patients.
Advancements in surgical techniques allow for more distal tumor resections.
Initial experiences with a flexible shaft stapler for very deep rectal cancers are presented.

Purpose of the Study:

To evaluate the efficacy and safety of a flexible shaft stapler for deep anterior rectal resection.
To assess oncological outcomes and functional results in patients undergoing this procedure.

Main Methods:

Six patients with very deep rectal cancer underwent resection using a flexible shaft stapler (Proximate AX55 mm).
The procedure involved rectum mobilization, stapler positioning with a curved flexible shaft, and firing.
Rectal resection was completed with an EEA stapled anastomosis.

Main Results:

Excellent outcomes observed in all six patients 3-18 months post-procedure.
No instances of cancer recurrence were reported.
Physiologic sphincter function was confirmed in two patients, with bowel movement frequency stabilizing between 2-8 daily.

Conclusions:

The transabdominal stapled method provides a safe and effective approach for distal rectal resection.
This technique ensures oncological safety while preserving sphincter function.
Improved quality of life is a key benefit for patients undergoing this procedure.
