Background:

  • Orthostatic tremor (OT) is characterized by leg tremor during standing.
  • Debate exists whether OT is a distinct syndrome or a variant of essential tremor (ET).

Observation:

  • Two patients with OT underwent comprehensive electrophysiological testing.
  • Tremor characteristics, including frequency and response to maneuvers, were assessed.
  • Novel accelerometric recordings in legs revealed tremor frequencies matching electromyographic bursts.

Findings:

  • Rapid (15-17 Hz) lower-extremity tremor bursts occurred only when standing.
  • Upper-extremity tremor bursts were synchronous with lower-extremity bursts.
  • Synchronous accelerometric and electromyographic frequencies, lack of family history, and response to clonazepam suggest OT is distinct from ET.
