Summary

Dobutamine enhances diastolic suction by increasing left ventricular restoring forces, independent of end-systolic volume. This effect may be linked to increased coronary blood volume.

Area of Science:

  • Cardiology
  • Physiology

Background:

  • Restoring forces generate diastolic suction during left ventricular contraction below equilibrium volume (Veq).
  • These forces are typically inversely related to end-systolic volume (ESV).
  • Previous studies suggest increased contractility augments restoring forces independently of ESV when the mitral valve is replaced.

Purpose of the Study:

  • To quantify left ventricular restoring forces in the presence of an intact mitral valve.
  • To investigate the effect of dobutamine on restoring forces and their relationship with ESV.
  • To explore the role of coronary blood volume in these effects.

Main Methods:

  • Utilized open-chest dogs (n=6) with an intact mitral valve.
  • Measured restoring forces as fully relaxed pressure (FRP) during nonfilling diastoles.

Related Experiment Videos

  • Administered dobutamine intravenously and via left anterior descending coronary artery infusion.
  • Related FRP to ESV under control and dobutamine conditions.
  • Main Results:

    • Dobutamine administration (IV and IC) caused a downward and rightward shift in the FRP-ESV relation.
    • This shift indicates augmented restoring forces at any ESV less than Veq.
    • Dobutamine slightly increased wall thickness (<10%) during nonfilling diastoles, suggesting increased coronary blood volume.

    Conclusions:

    • Dobutamine increases left ventricular restoring forces independently of ESV changes.
    • Dobutamine also increases Veq.
    • Increased coronary blood volume may contribute to the enhanced restoring forces observed with dobutamine.
