Background:

  • Adenoidectomy is a long-standing treatment for suppurative otitis media.
  • The role of adenoids in otitis media with effusion (OME) requires further investigation.

Purpose of the Study:

  • To investigate the role of adenoids in the pathogenesis of otitis media with effusion (OME).
  • To analyze adenoid tissue from OME patients using advanced microscopy and immunological techniques.

Main Methods:

  • Light microscopy, immunocytochemistry, enzyme cytochemistry, and electron microscopy were employed.
  • Adenoid specimens from 28 children with OME and 10 controls were examined blindly.
  • Quantitative analysis of antigen-presenting cells was performed.

  • Increased lymphocytes, mast cells, plasma cells, macrophages, dendritic cells, and M cells were observed in OME patients' adenoids.
  • Stratified squamous epithelial areas, collagenous fibers, and fibrocytes were also elevated in the OME group.
  • Major histocompatibility complex (MHC) class II positivity indicated antigen-presenting functions in some epithelial cells of OME patients.

Conclusions:

  • Adenoid tissues in OME patients appear to function as infectious foci.
  • These tissues may exacerbate immune reactions that ascend to the middle ear.
  • Findings suggest a significant role for adenoids in the development and persistence of OME.
