Hydroxyurea, used for blood disorders, can rarely cause painful leg ulcers. Stopping the medication helped these ulcers heal in two patients.

Area of Science:

  • Hematology
  • Dermatology
  • Oncology

Background:

  • Hydroxyurea is a standard treatment for myeloproliferative disorders like CML and polycythemia vera.
  • Common side effects include hair loss, skin darkening, and nail changes, particularly with prolonged use.
  • Leg ulcers are a rare but serious adverse effect of hydroxyurea therapy.

Observation:

  • Two patients receiving long-term hydroxyurea for myeloproliferative neoplasms developed spontaneous, painful leg ulcers.
  • These ulcers were resistant to conventional treatment while hydroxyurea therapy continued.

Findings:

  • The development of painful leg ulcers was directly associated with long-term hydroxyurea treatment.
  • Cessation of hydroxyurea resulted in the healing of these ulcers in both reported cases.

Related Experiment Videos

Implications:

  • Clinicians should consider hydroxyurea-induced leg ulcers in patients with myeloproliferative disorders presenting with unexplained leg ulcers.
  • Discontinuation of hydroxyurea may be necessary for the resolution of these ulcers.
  • Further research is warranted to understand the mechanism and incidence of hydroxyurea-associated leg ulcers.
