    Summary

    In hemorrhagic shock, kidneys significantly produce glucose. However, in endotoxic shock, increased glucose utilization by blood components, not kidneys, causes hypoglycemia.

    Background:

    • Renal gluconeogenesis and systemic glucose utilization are key regulatory factors.
    • Hemorrhagic and endotoxic shock present distinct metabolic challenges.

    Purpose of the Study:

    • To investigate the roles of renal gluconeogenesis and glucose utilization in hemorrhagic and endotoxic shock.
    • To determine the primary drivers of hypoglycemia in these shock states.
    • To identify tissues responsible for increased glucose demand during endotoxic shock.

    Main Methods:

    • Anesthetized, eviscerated dogs (nephrectomized and non-nephrectomized) were subjected to hemorrhage or endotoxin injection.
    • Blood glucose levels and glucose utilization rates were measured.
    • In vitro experiments assessed endotoxin's effect on blood glucose utilization.

    Main Results:

    • Glucose utilization increased in both hemorrhagic and endotoxic shock, most notably after endotoxin.
    • Kidneys contributed to glucose production in hemorrhagic shock, indicated by greater glucose drop in nephrectomized animals.
    • Kidneys did not appear to perform gluconeogenesis in endotoxic shock.
    • In vitro studies showed endotoxin-treated blood exhibited increased glucose utilization, comparable to eviscerated animals.

    Conclusions:

    • The kidneys play a significant gluconeogenic role in hemorrhagic shock.
    • In endotoxic shock, elevated glucose demand by blood components, rather than impaired renal function, contributes to lethal hypoglycemia.
    • These findings highlight the complex metabolic derangements in critical illness and shock states.
