In secondary hyperparathyroidism (secondary HPT), parathyroid glands maintain normal calcium sensing but have a blunted response to calcium, unlike primary HPT. This impaired calcium inhibition of PTH release is linked to increased parathyroid gland mass, not an early sensing defect.
