Summary

Survival rates for penetrating cardiac injuries are low overall, with gunshot wounds having a 9.7% survival rate and stab wounds at 32.6%. Hospital survival rates are higher but show similar trends for these traumatic injuries.

Area of Science:

  • Trauma Surgery
  • Emergency Medicine
  • Public Health

Background:

  • Penetrating cardiac injuries have variable reported survival rates due to reliance on hospital-based data.
  • Local trauma system variations influence reported outcomes for cardiac trauma.

Purpose of the Study:

  • To determine population-based and hospital-based survival rates for penetrating cardiac injuries.
  • To provide a more accurate assessment of outcomes for penetrating cardiac injuries.

Main Methods:

  • Retrospective cohort analysis of 20,181 trauma admissions and 6,492 medical examiner reports over 7 years.
  • Meta-analysis comparing study survival rates with existing population-based reports.
  • Inclusion of all penetrating cardiac injuries within the study period.

Related Experiment Videos

Main Results:

  • Identified 212 penetrating cardiac injuries, with an incidence of 1 per 100,000 man-years.
  • Overall population survival rate was 19.3% (41/212); gunshot wounds: 9.7% (12/123), stab wounds: 32.6% (29/89).
  • Hospital survival rate was 42.7% (41/96); gunshot wounds: 29.3% (12/41), stab wounds: 52.7% (29/55).

Conclusions:

  • Population-based data indicate minimal improvement in survival rates for penetrating cardiac injuries.
  • Distinguishing between gunshot and stab wounds is crucial for understanding survival outcomes.
