Summary

Pertussis is common in adults due to waning immunity, making them a reservoir for the disease. Booster immunization with acellular vaccines could eliminate pertussis in all age groups.

Area of Science:

  • Infectious Diseases
  • Immunology
  • Public Health

Background:

  • Pertussis (whooping cough) is a vaccine-preventable illness that frequently affects adults.
  • Adults are a significant reservoir for pertussis transmission in the United States, often unrecognized by clinicians.
  • Waning immunity contributes to the high prevalence of pertussis in the adult population.

Purpose of the Study:

  • To review the epidemiology, clinical presentation, diagnosis, and treatment of pertussis in adults.
  • To highlight the role of adults as a reservoir for pertussis.
  • To discuss the potential impact of adult booster immunization on disease elimination.

Main Methods:

  • A comprehensive MEDLINE search of English-language articles on pertussis in adults was performed.
  • Relevant findings from the literature search were synthesized into a narrative review format.

Main Results:

  • Pertussis commonly presents as a prolonged cough illness in adults, often following upper respiratory symptoms.
  • Sporadic cases and occasional outbreaks occur in diverse settings, including educational institutions and healthcare facilities.
  • The review covers diagnostic approaches and therapeutic strategies for adult pertussis.

Conclusions:

  • Waning immunity necessitates considering pertussis in adult patients presenting with prolonged cough.
  • Routine booster immunization for adults using acellular pertussis vaccines is proposed as a strategy to control and potentially eliminate the disease in both children and adults.
