    Summary

    Benign cerebellar astrocytomas rarely spread to the spinal subarachnoid space in children. Despite their low grade, aggressive treatment is recommended for these rare cases of tumor seeding.

    Background:

    • Cerebellar astrocytomas are common pediatric brain tumors.
    • Typically, these tumors exhibit slow growth and have a good prognosis.
    • Spinal subarachnoid seeding is an uncommon event for cerebellar astrocytomas.

    Observation:

    • Presents three pediatric cases of cerebellar astrocytoma with spinal subarachnoid dissemination.
    • Histological confirmation of tumor similarity between the posterior fossa and spinal implants in two patients.
    • All cerebellar tumors were histologically benign (WHO Grade I).

    Findings:

    • Subarachnoid seeding from benign cerebellar astrocytomas is exceptionally rare.
    • Tumor behavior, apart from seeding, was indolent in the presented cases.
    • Literature review revealed no similar documented cases of spinal seeding from cerebellar astrocytomas.

    Implications:

    • Highlights an unusual metastatic pattern for a typically benign tumor.
    • Suggests the need for vigilance regarding potential dissemination even in low-grade cerebellar astrocytomas.
    • Advocates for aggressive therapeutic strategies in pediatric patients with subarachnoid seeding from cerebellar astrocytoma to improve outcomes.
