- Patients with left main (LM) or triple-vessel (3V) CAD exhibited statistically significant higher lung-to-heart (L:H) ratios (p < 0.001) compared to patients with 0-2 significantly stenosed coronary arteries, irrespective of stress method (HDD or EST).
- While L:H ratios tended to be higher with HDD stress, this difference was not statistically significant (p = NS) between the HDD and EST groups.
- Increased L:H ratios demonstrated a strong correlation with the presence of LM/3V CAD.

- Lung-to-heart (L:H) ratios derived from Tl-201 SPECT imaging are valuable markers for detecting left main (LM) or triple-vessel (3V) coronary artery disease (CAD).
- Both high-dose dipyridamole (HDD) and exercise stress (EST) can be effectively utilized with L:H ratio analysis for CAD severity assessment.
- The combination of SPECT imaging, L:H ratios, and particularly HDD stress, holds significant potential for enhancing the diagnostic accuracy of coronary artery disease evaluation.
