Cardiac transplantation: three decades of experience define our challenge

By defining some issues of controversy surrounding cardiac transplantation in its contemporaneous practice, we can set the stage for clinical trials that explore further these issues and bring reason based on clinical trial evidence to our daily practices. As we enter the twenty-first century we will surely have better insight into patient selection and post heart transplant patient management if we can combine our talents and resources to perform these carefully designed clinical efforts.
