Summary

Plasma endothelin-1 (ET-1) levels are elevated during the early phase of migraine attacks. These potent vasoconstrictors may play a role in migraine-associated vasoconstriction, potentially linked to serotonin activation.

Background:

  • Endothelins (ETs) are potent vasoconstrictors.
  • ETs are implicated as potential mediators in migraine pathophysiology.
  • The role of ET-1 in acute migraine attacks requires further elucidation.

Purpose of the Study:

  • To investigate plasma endothelin-1 (ET-1) levels in migraine patients.
  • To compare ET-1 levels during ictal (attack) and interictal (between attacks) phases.
  • To explore the temporal dynamics of ET-1 during a migraine attack.

Main Methods:

  • Plasma ET-1 levels were measured in 31 migraine patients (9 with aura, 22 without aura).
  • Measurements were taken during both ictal and interictal periods.
  • Ictal measurements were further categorized by time elapsed since migraine onset (<2h and 4-6h).

Main Results:

  • Mean interictal plasma ET-1 was 5.3 pg/ml (SD 1.8).
  • Mean ictal plasma ET-1 was 6.4 pg/ml (SD 3.9).
  • Ictal ET-1 levels were significantly elevated early in the migraine attack (<2 hours) and decreased later (4-6 hours).

Conclusions:

  • Elevated plasma ET-1 levels are observed during the early stages of migraine attacks.
  • Endothelin-1 may contribute to the vasoconstrictive phase of migraine.
  • Serotonin activation could be a secondary factor leading to ET-mediated vasoconstriction in migraine.
