Background:

Composite auricular defects, ranging from one-fourth to half of the ear, often result from trauma or inflammation.
Current reconstructive methods may involve multiple stages or result in suboptimal aesthetic outcomes.

Observation:

A novel one-stage surgical approach was developed utilizing combined postauricular skin and mastoid fascial flaps.
This technique provides bilateral coverage for a grafted framework in auricular reconstruction.

Findings:

The described flap technique proved safe and effective for repairing moderate auricular composite defects.
The procedure resulted in minimal scarring at the postauricular donor site.
Excellent aesthetic results were achieved, restoring natural ear shape, size, and cartilage convolution.
