Summary

Transconjunctival lower eyelid blepharoplasty can cause double vision (diplopia) due to injury to eye muscles. Surgeons must understand eyelid anatomy to avoid these complications during cosmetic surgery.

Background:

  • Transconjunctival blepharoplasty for lower eyelid fat removal is increasingly popular.
  • Complications, particularly diplopia, are underreported in the literature.

Observation:

  • Six patients developed diplopia after transconjunctival lower eyelid blepharoplasty.
  • Injuries involved inferior rectus and inferior oblique muscles in 4/6 patients, and lateral rectus in one.

Findings:

  • Mechanisms include muscle hemorrhage, edema, scarring, or incorporation into closure.
  • Two patients needed strabismus surgery; four improved with observation.

Implications:

  • Surgeons need thorough knowledge of eyelid anatomy for the transconjunctival approach.
