Background:

  • Traditional arch bar maxillomandibular fixation (MMF) for mandibular fractures presents risks including glove perforation, blood-borne pathogen exposure, and dental/gingival injury.
  • Safer alternatives to arch bar MMF are needed to mitigate these risks and improve patient care.

Observation:

  • A prospective study evaluated orthodontic direct bonded bracket fixation (MMF/DBB) in 14 patients with mandibular fractures.
  • Patients presented with various fracture types, including symphysis, angle, condylar neck, coronoid, and body fractures.
  • MMF/DBB was used alone in nine patients and in conjunction with open reduction/internal fixation (ORIF) in five patients.

Findings:

  • No instances of infection, malocclusion, malunion/nonunion, or enamel/gingival injury were reported.

  • Patients treated with MMF/DBB demonstrated superior oral hygiene compared to historical controls using arch bars.
