Dexfenfluramine effectively treated severe overeating in three psychotic patients. This medication managed binge eating without worsening psychiatric symptoms, even in complex cases.

Background:

  • Dexfenfluramine (DF) is contraindicated in patients with severe psychiatric disorders.
  • Severe overeating can co-occur with chronic psychosis, presenting complex treatment challenges.

Observation:

  • Three patients with chronic psychosis and severe overeating were treated with dexfenfluramine (DF).
  • Psychiatric pharmacotherapy remained unchanged during DF administration.
  • Patients included those with paranoid schizophrenic psychosis and mixed psychosis.

Findings:

  • Overeating was resolved in all three patients without adverse effects on their psychotic states.
  • Body weight was maintained by all patients.
  • Two patients with poorly controlled diabetes showed significant metabolic improvements.
  • Dosage of up to 75 mg/day was required in one patient during binge eating episodes.

Implications:

  • Dexfenfluramine (DF) may be a viable treatment for severe overeating in select psychotic patients.
  • Close psychiatric supervision is crucial when using DF in this population.
  • Further research is warranted to explore the efficacy and safety of DF in psychotic patients with eating disorders.
