Summary

Regular exercise may help maintain bone mineral density (BMD) in women with bulimia nervosa, but not in those with anorexia nervosa. Weight-bearing activity appears crucial for preventing bone loss in bulimic patients.

Background:

  • Eating disorders, including anorexia nervosa and bulimia nervosa, are associated with significant health complications.
  • Menstrual dysfunction and low bone mineral density (BMD) are common in patients with eating disorders.
  • The role of exercise in mitigating these complications requires further investigation.

Purpose of the Study:

  • To investigate the relationship between exercise, menstrual function, and BMD in age-matched patients with eating disorders (anorexia nervosa, bulimia nervosa) and healthy controls.
  • To determine if exercise can attenuate bone loss in these patient groups.

Main Methods:

  • A clinical interview assessed dieting, eating disorder history, physical activity, and menstrual history in 43 bulimic patients, 13 anorectic patients, and 17 healthy controls (ages 18-29).
  • Bone mineral density (BMD) was measured using dual x-ray absorptiometry.
  • Participants were categorized based on exercise habits (regular, intermittent, sedentary).

Main Results:

  • Anorectic patients exhibited significantly lower BMD across all measured skeletal regions compared to bulimic patients and controls.
  • Bulimic patients who engaged in regular weight-bearing exercise showed higher total body BMD than sedentary bulimics.
  • Exercise was associated with improved BMD in the lumbar vertebrae, femoral neck, and legs of bulimic patients compared to sedentary individuals.

Conclusions:

  • Weight-bearing exercise may prevent or reduce bone loss at specific skeletal sites in normal-weight bulimic patients.
  • Exercise does not appear to prevent bone loss in patients with anorexia nervosa.
  • Menstrual function history did not show a significant relationship with BMD within exercise subgroups of bulimic patients.
