Summary

A rare case of a giant vesico-vaginal stone in a 74-year-old female with cerebral palsy was treated. The massive 435g stone, occupying the bladder and vagina, was successfully removed via cystolithotomy.

Background:

  • A 74-year-old female with lifelong total incontinence due to cerebral palsy presented with lower abdominal and anal pain.
  • Physical examination revealed a palpable giant stone on vaginal examination.

Observation:

  • Radiography showed a large pelvic calcification.
  • Magnetic resonance imaging (MRI) confirmed a giant vesico-vaginal stone occupying most of the bladder and vagina.

Findings:

  • Cystolithotomy was performed to remove the stone.
  • The surgically removed stone weighed 435g, measuring 9.0 x 6.5 x 5.5 cm, and was composed of magnesium ammonium phosphate.
  • This represents one of the few reported cases of giant vesical stones in females.

Implications:

  • This case highlights a rare presentation of a giant vesico-vaginal stone.
  • The stone was unrelated to gynecological procedures, suggesting other etiologies.
  • Successful surgical intervention (cystolithotomy) is feasible for such large stones.
