Recurring gastric erosions, or aphthous ulcers, caused persistent dyspeptic symptoms in five patients. Gastric surgery (highly selective vagotomy) successfully relieved symptoms and healed ulcers when conventional therapies failed.

    Area of Science:

    • Gastroenterology
    • Surgical Gastroenterology

    Background:

    • Dyspeptic symptoms and negative barium meals prompted repeated endoscopic evaluation.
    • Recurring gastric erosions, termed aphthous ulcers, were identified as the underlying cause.

    Observation:

    • Five patients presented with persistent dyspeptic symptoms despite negative initial barium meal investigations.
    • Multiple recurring gastric erosions (aphthous ulcers) were diagnosed via endoscopy.
    • Superficial chronic gastritis was noted in four patients, but no common etiological factor was identified.

    Findings:

    • Standard peptic ulcer treatments were ineffective in managing symptoms and ulceration.
    • Two patients underwent highly selective vagotomy, leading to symptom resolution and ulcer healing.

    Implications:

    • Highly selective vagotomy may be an effective treatment for refractory gastric aphthous ulcers.
    • This suggests a distinct clinical entity requiring consideration beyond typical peptic ulcer disease.
