Summary

Cartridge cracks during foldable intraocular lens (IOL) insertion were common (14.86%), particularly when using sodium hyaluronate. Using chondroitin-based agents significantly reduced these cracks, improving IOL delivery safety.

Area of Science:

  • Ophthalmology
  • Surgical Technology

Background:

  • Foldable intraocular lenses (IOLs) are standard in cataract surgery.
  • IOL insertion can be complicated by device malfunctions.

Purpose of the Study:

  • To determine the incidence of cartridge cracks during foldable IOL insertion.
  • To identify factors contributing to cartridge cracks.

Main Methods:

  • 350 small incision cataract surgeries using the SI-40 foldable IOL.
  • Evaluation of three viscoelastic agents: sodium hyaluronate (Healon, Vitrax) and sodium chondroitin sulfate-sodium hyaluronate (Viscoat).

Main Results:

  • Cartridge cracks occurred in 14.86% of cases.
  • 98.1% of cracks happened when using Healon.

Related Experiment Videos

  • Plunger overriding the optic edge was observed in cracked cartridges.
  • Conclusions:

    • Chondroitin-based viscoelastic agents are recommended for loading the SI-40 IOL to minimize cartridge cracks.
    • Potential for IOL inserter design modifications to resolve cartridge cracking issues.
