Background:
- Perinuclear antineutrophil cytoplasmatic autoantibodies (p-ANCA) in ulcerative colitis (UC) have inconsistent data regarding their target antigens and pathophysiological relevance.
- Previous studies often investigated only a single potential target antigen for p-ANCA in UC.
Purpose of the Study:
- To simultaneously assess five different p-ANCA subtypes in UC patients.
- To determine reactivity against specific target antigens.
- To correlate p-ANCA specificities with clinical features of UC and current immunosuppressive therapy.

- Sera from 61 UC patients and 56 healthy controls were tested.
- Indirect immunofluorescence and enzyme-linked immunosorbent assays were used for elastase, lactoferrin, cathepsin G, myeloperoxidase, and bactericidal permeability increasing protein (BPI).
- p-ANCA subtypes were correlated with disease extent, extraintestinal manifestations, and immunosuppressive therapy.
Main Results:
- p-ANCA detected in 46% of UC patients and 4% of controls.
- p-ANCA subtype distribution: anti-BPI (26%), anticathepsin G (16%), antielastase (15%), antilactoferrin (7%), antimyeloperoxidase (5%).
- Anticathepsin G antibodies showed a negative correlation with immunosuppressive therapy; no other correlations with clinical characteristics were found.
Conclusions:
- p-ANCA subtypes in inflammatory bowel disease (IBD) react with diverse target antigens.
- p-ANCA specificities in UC are not correlated with clinical features of the disease.
