Isolated sphenoethmoid recess (SER) polyps, often from the sphenoid sinus, can indicate underlying pathology. Pre-operative biopsy is crucial for guiding transnasal endoscopic surgery in these rare cases.

Background:

Sphenoethmoid recess (SER) polyps are uncommon and typically originate in the sphenoid sinus.
These polyps can be the sole presenting clinical abnormality.

Observation:

Six patients with isolated SER polyps underwent investigation including biopsy and CT/MR imaging.
Histology revealed inflammatory disease (3), inverted papilloma (2), and ectopic pituitary adenoma (1).

Findings:

One inflammatory polyp originated from the sphenoid ostium mucosa; five involved the sphenoid sinus.
Transnasal endoscopic surgery was effective for all polyps except the pituitary adenoma.

Implications:

Isolated SER polyps warrant investigation for underlying sphenoid sinus pathology.
