Area of Science:

  • Otolaryngology
  • Infectious Diseases
  • Pathology

Background:

  • Neck masses and discharging sinuses can indicate various pathologies, including infections.
  • Tuberculosis (TB) is a significant global health concern, though cervical presentations are less common.

Observation:

  • A 55-year-old male presented with progressive, painless neck swelling and multiple discharging sinuses.
  • Radiography revealed a dense radio-opaque shadow in the neck, with a serum calcium level of 2 mmol/l.
  • Surgical exploration identified an irregular calculus near the left carotid sheath.

Findings:

  • Histopathological examination confirmed tuberculosis as the cause of the cervical lesion.
  • The calculus was associated with the tuberculous process, likely a reactive or calcified lymph node.

Implications:

  • This case highlights the importance of considering tuberculosis in the differential diagnosis of neck masses with sinus tracts, even in the absence of typical systemic symptoms.
  • Early diagnosis and appropriate anti-TB treatment are crucial for successful management and patient outcomes.
  • Cervical tuberculous lymphadenitis can present with unusual radiological and anatomical findings.
